What a consent form involves

A consent form records a person's agreement to a specific processing of their personal data: a photo release, a marketing list, a medical record, a background check. Organizations issue the same approved form to every person, filled with that person's details.

The consent wording is reviewed once by whoever is responsible for data protection and then must stay stable. Only the data subject's identity and the specifics of the processing instance change per copy.

The data that gets filled in

• Data subject: full name, address, date of birth or ID number
• The specific processing consented to
• The controller's name as your form states it
• Date and place of signing

The values usually arrive in:

• A photo or scan of the person's ID
• The registration form or email with the person's details
